Hi, the book you are looking for is now out of print but james balch and
his 
wife phyllis have wrote a book called 'PRESCRIPTION FOR NUTRITIONAL
HEALING' 
and its available from Amazon.co.uk or amazon.com.
Look out for the used books they tend to be cheaper.
